DoD Forms Management Program

The Department of Defense (DoD) Forms Management Program is administered by the Directives Division (DD), Executive Services Directorate, Washington Headquarters Services. In coordination with the DoD, CIO, the DD establishes policies and procedures for the DoD Forms Management Program, including responsibilities for reviewing and approving form requirements. DoD forms satisfy a valid need; essential to accomplish a mission and necessary for the efficient and economical operation of the DoD.

This web site provides the “Official” edition of Department of Defense (DD) and Secretary of Defense (SD) forms. The DoD Forms Management Program web site serves as the single source of information for forms belonging to and used with the Department of Defense in accordance with “Web Site Administrative Policies and Procedures.”
