Ideas and Sources Reference Material
Use the Ideas and Sources Reference manual in 2 ways: First, score
your survey using 1 of the 4 scoring sheets provided for you.
Make sure to check which survey you completed (either climate
survey or survey). Then check which version you completed (red
or blue). Use the correctly labeled scoring sheet to find out
what you think about quality practices in your organization.
After you've scored your survey, use the reference material to find out what you can do to improve quality performance in your organization. Reference material page numbers are located next to the scores for each quality category.
For each category, you will obtain a mean score by adding the response numbers you've circled for a set of questions and dividing by the total number of questions. You will not count questions that you may have responded to with N/A.
For example, using the Vision for the Future category, assume that you circled N/A as your response to question 25. Let's also assume you circled a "4" for question 33 and a "5" for question 54. Add these responses numbers (4 + 5 = 9) and enter the 9 in the appropriate box. Place the number of questions you answered excluding N/A in the appropriate box. In this case, the number is 2. Then divide the sum of the responses numbers by 2 (9 2 = 4.50) and enter the 4.50 in the appropriate box. If you had answered all 3 questions, you would add the response numbers for all 3 questions and divide by 3.
One question requires "reverse" scoring. Question 12 is marked with an asterisk and appears in bold. (See Communications example below). This question is worded negatively, thus a response of "6" is scored as "1", a response of "5" as "2", "4" as "3", "3" as "4", "2" as "5", and "1" as "6". Use the corrected score as the response number for those questions.
In all cases, carry out division to 2 decimal places.
